The giveaway (the "Giveaway") is sponsored by Total Care Wellness LLC, doing business as Living Well with Dr. Michelle, 9912 N 4700 W, Suite 400, Cedar Hills, UT 84062 ("Sponsor").
The Giveaway is open only to legal residents of the State of Utah who are 18 years of age or older at the time of entry. Employees, contractors and owners of Sponsor and of Total Care Dental and Total Care Kids, and members of their immediate families (spouse, parents, children, siblings) and households, are not eligible.
The Giveaway begins at 12:00 AM Mountain Time (MT) on October 18, 2026 and ends at 11:59 PM MT on November 9, 2026 (the "Entry Period"). Sponsor's computer is the official clock.
Visit livingwellwithdrmichelle.com/pages/utah-giveaway during the Entry Period and complete the entry form with your first name, a valid email address and your Utah ZIP code. Limit one (1) entry per person and per email address. Additional entries are not counted. Entries made by automated means, or through multiple or false email addresses, are void. By entering, you agree to receive marketing emails from Sponsor; you may unsubscribe at any time, and unsubscribing does not affect your entry.
One (1) grand prize winner will receive: one Tooth Powder 1 Year Supply (four 1.8 oz jars, winner's choice of formula) and eight (8) bottles of Remineralizing Mouthwash (winner's choice of available flavor). Approximate retail value (ARV): $266. The prize must be picked up in person at Sponsor's store, 9912 N 4700 W, Suite 400, Cedar Hills, UT 84062, during store hours (Monday to Friday, 10:00 AM to 4:00 PM) by December 31, 2026. The prize is not transferable, and no cash or other substitution is allowed, except that Sponsor may substitute items of equal or greater value if an item becomes unavailable. Any taxes are the winner's responsibility.
Every eligible entrant will receive by email one single-use code for 20% off a purchase at Sponsor's Cedar Hills store. The code is valid for in-store purchases only through 11:59 PM MT on the 14th day after the entry is received (or the following Monday, if that day falls on a Saturday or Sunday), as stated in the email, has no cash value, cannot be combined with other coupon codes, and is limited to one per person. Codes are void if sold, shared publicly or obtained through ineligible entries.
The winner will be selected in a random drawing from all eligible entries on or about November 10, 2026. Odds of winning depend on the number of eligible entries received. The potential winner will be notified by the email address used to enter and must reply within seven (7) days to claim the prize. If the potential winner does not respond in time, is found ineligible, or does not comply with these rules, the prize may be forfeited and an alternate winner selected at random.
By entering, entrants agree to release and hold harmless Sponsor and its owners, employees and agents from any claims, losses or damages arising from participation in the Giveaway or the acceptance or use of any prize.
Information collected from entrants is used to administer the Giveaway and to send emails from Sponsor, as described in our privacy policy.
The Giveaway is governed by the laws of the State of Utah.
This Giveaway is in no way sponsored, endorsed or administered by, or associated with, Meta Platforms, Inc., Facebook or Instagram. Entrants release Meta, Facebook and Instagram from any liability related to the Giveaway.
